What is INUSpheresis®?

INUSpheresis® is a form of therapeutic apheresis – a procedure that filters the blood outside the body and returns it. It uses double filtration plasmapheresis, sometimes shortened to DFPP.

 

The process runs in two stages. A first filter separates plasma from your blood cells. A second, finer filter then passes that plasma through a membrane sized to hold back larger molecules while letting smaller ones through. The filtered plasma is recombined with your blood cells and returned to you.

 

What distinguishes it from a simple plasma exchange is that stage two is selective. Rather than discarding plasma wholesale and replacing it with donor plasma or albumin, the second filter is chosen to retain useful plasma components while removing larger targeted ones.

How INUSpheresis® differs from other treatments

This is the question we are asked most often, so here is a direct answer.

 

Compared with an IV drip or vitamin infusion:
An infusion adds something to your bloodstream. INUSpheresis® removes something from it. They work in opposite directions and are not alternatives to one another.

 

Compared with dialysis:
Dialysis substitutes for kidney function, filtering small waste molecules the kidneys can no longer clear, and is prescribed for renal failure. INUSpheresis® does not replace organ function and is not a treatment for kidney disease. The two share the general principle of filtering blood outside the body and very little else.

 

Compared with conventional plasma exchange: 
Plasma exchange removes plasma and replaces the volume with donor plasma or albumin. INUSpheresis® filters your own plasma and returns it, which removes the need for replacement fluid.

 

Compared with ozone or oxygen therapies: 
These aim to change how blood behaves once it is back in circulation. INUSpheresis® is a physical filtration process. Some patients undergo both, but they are doing different jobs.


Compared with chelation: 
Chelation uses a pharmaceutical agent that binds to certain metals so they can be excreted. INUSpheresis® is mechanical rather than pharmacological. It does not involve a chelating drug.

What happens during treatment

Before the session
You attend a consultation first. This includes a medical history, a physical assessment and blood work. Nothing is booked until that assessment is complete and our clinician has confirmed the treatment is appropriate for you.
Two intravenous lines are placed, usually in the crease of each arm. The sensation is comparable to giving blood.
You recline for the duration while your blood circulates through the filtration system and back. The session typically takes two to three hours. Most people read, work or sleep. A member of the clinical team monitors you throughout and the session can be paused or stopped at any point.
You rest briefly, then leave the same day. It is an outpatient procedure. We ask that you arrange for someone to drive you home from your first session.
There is no standard number. Some people are advised on a single session, others on a short course. This is determined by your clinician after assessment and reviewed as you go – we do not sell fixed packages of sessions in advance.

What the filtration is designed to remove

The second-stage filter is selected by molecular size. In broad terms it is intended to retain larger molecules from the plasma while allowing smaller ones to pass back through. Depending on the filter selected, the categories targeted include inflammatory proteins, immune complexes, autoantibodies, certain lipoproteins, and larger environmental and metabolic compounds.

 

We deliberately do not publish removal percentages for individual substances. Figures of that kind depend heavily on the filter used, the individual patient, the number of passes and the analytical method, and presenting them as a general expectation would be misleading.

 

What is measurable is what comes out. The filtrate collected during your session can be analysed, and your clinician will discuss those findings with you afterwards.

Being straight with you about the evidence

Most clinics offering this treatment do not publish this section. We think you should have it before you decide.

 

Therapeutic apheresis is a well-established procedure with a substantial evidence base in specific clinical settings, and it has been in hospital use for many years. That part is not in question.

 

The evidence for apheresis used as a general wellness, detoxification or longevity intervention is at a much earlier stage. The published work in that area consists largely of case reports, small observational studies and clinical experience rather than large randomised controlled trials. It is reasonable to describe it as emerging, and unreasonable to describe it as proven.

 

That does not mean it has no value. It means the honest position is that we cannot tell you in advance what result you will get, and any clinic that does tell you that is going beyond what the evidence supports.

 

Our approach follows from this. We assess first. We are clear about what is known and what is not. We will decline to treat where we do not think there is a sound reason to proceed, and we would rather do that than take a booking.

 

Screening and assessment: Every patient is screened before treatment. This covers cardiovascular health, blood counts and clotting, current medications, infection status, venous access and pregnancy. Where screening identifies a reason not to proceed, we say so.

Preparation and aftercare

Preparing for your session, and what follows

In the days before
Stay well hydrated and eat normally. Your clinician will advise on any medication that needs adjusting – do not stop anything on your own. Avoid alcohol for 24 hours beforehand.
Have breakfast. Wear something with sleeves that push up easily. Bring anything you would like for two to three hours of sitting still.
Some people feel tired for the rest of the day. Others feel unchanged. Both are ordinary. Drink plenty of water and plan a quiet evening.
Any effects people report tend to emerge gradually rather than immediately. We will arrange a follow-up to review how you have responded and to discuss your filtrate analysis.

Is INUSpheresis® painful?
The filtration itself is not felt. You will feel the two needles going in, which is comparable to giving blood or having a cannula placed. Most people describe the session as uneventful and slightly boring, which is what we are aiming for.
Two to three hours for the filtration, plus time either side for preparation and a short rest afterwards. Allow half a day for your first appointment. Follow-up sessions, if advised, tend to run more quickly because you know what to expect.
No. Dialysis substitutes for kidney function in people whose kidneys have failed, and clears small waste molecules. INUSpheresis® does not replace organ function and is not a kidney treatment. Both filter blood outside the body, and that is where the similarity ends.
